This standard specifies the scope of road traffic injured accident disability assessment principles, methods and content.
This standard applies to persons injured in road traffic assessment of disability.
2 Review terms and definitions

3 General

4 grade level
4.1 Ⅰ brain disability
4.1.1 , spinal cord and peripheral nerve injury caused by:
a) vegetative state;
b) extreme mental impairment (IQ below 20) or psychiatric disorder, daily life can not take care of themselves;
c) quadriplegia (three or more limbs less muscle strength grade 3);
d) paraplegia (muscle strength grade 2 or less) with fecal and urinary incontinence.
4.1.2 Face and Head Injury:
a) bilateral eye missing;
b) side of the missing eye, the other side of the blind eyes with severe deformities 5.
4.1.3 thoracic spinal deformity due to serious injury healing, breathing with severe dysfunction.
4.1.4 neck injury caused by breathing and swallowing severe dysfunction.
4.1.5 Chest Injury:
a) extensive lobectomy or bilateral pleural adhesions or severe thoracic major malformations, severe respiratory disorders;
b) heart function insufficiency, cardiac function Ⅳ grade; or heart failure, heart arrhythmia grade Ⅲ with significant organic.
4.1.6 Abdominal Injury:
a) the stomach, intestine, digestive gland and other parts removed a serious obstacle to digestion and absorption , daily life can not take care of themselves;
b) bilateral nephrectomy or total loss of life can not take care of themselves.
4.1.7 Physical Injury:
a) three or more missing limbs (upper limb in the wrist more joints, lower extremity above the ankle);
b) missing the second leg (above the elbow joint upper and lower limbs above the knee), and another 50% loss of function of limbs;
c) two limbs missing ( upper limb than in the wrist, leg above the ankle), the third complete loss of limb function;
d) lack of a limb (arm above the elbow, legs above the ankles), the second complete loss of limb function, the first 50% loss of function of three or more limbs;
e) loss of a limb (upper limb above the wrist, leg above the ankle joint), complete loss of function of the other two limbs;
f) total loss of three limbs.
4.1.8 skin damage caused by scarring of body surface area of more than 76%.
4.2 Ⅱ level disability
4.2.1 brain, spinal cord and peripheral nerve damage caused by:
a) severe intellectual impairment (IQ 34 below) or psychiatric disorder, daily life, people need help at any time to complete;
b) complete aphasia;
c) both eyes blind 5;
d) quadriplegia (two or more limbs muscle strength two the following);
e) hemiplegia or paraplegia (level 2 strength).
4.2.2 head and face injuries caused by:
a) side of the missing eye, another eye blind 4; or the side of the eye missing, the other side of the blind eyes with severe deformity more than grade 3;
b) of severe bilateral eyelid ptosis (or severe deformity) with four or more blind eyes; or heavy eyelids drooping on one side (or major malformations), the eye blind 4 or above, the other eye blind 5;
c) both eyes blind 5;
d) extremely hearing-impaired ears with bilateral ear loss (or severe deformity); or binaural hearing disorder with a highly side of the ear missing, the other side of the ear severely deformed;
e) overall scar formation.
4.2.3 thoracic spinal deformity due to serious injury healing, respiratory dysfunction.
4.2.4 Neck Injury breathing and swallowing dysfunction.
4.2.5 Chest Injury:
a) the widespread and serious lung resection or pleural adhesions or thoracic deformity, respiratory dysfunction;
b) heart failure, cardiac function grade Ⅲ; or heart failure, cardiac arrhythmia grade Ⅱ with significant organic.
4.2.6 abdominal injuries caused by the side of the resection or complete loss of kidney function, the other side of the renal function and severe disorders.
4.2.7 limb Injury:
a) two limbs missing (upper limbs above the elbow, legs above the knee);
b) lack of a limb (arm above the elbow, legs above the knee), and the other limb complete loss of function;
c) total loss of more than two limbs.
4.2.8 scarring of the skin damage caused by more than 68% body surface area.
4.3 Ⅲ grade brain disability
4.3.1 , spinal cord and peripheral nerve injury caused by:
a) severe mental defect or mental disorder, can not live independently, care was often required;
b) severe traumatic epilepsy, drugs can not control, the average monthly grand mal or more, or the limitations of four or more episodes per month or less than seven seizures per week, or psychomotor seizures, more than three times per month;
c) bilateral severe facial paralysis, difficult to restore;
d) severe involuntary movements or ataxia;
e) quadriplegia (two limbs less than grade 3 strength);
f) hemiplegia or paraplegia (muscle strength grade 3 the following);
g) stool or urinary incontinence, and difficult to restore.
4.3.2 head and face injuries caused by:
a) side of the missing eye, another eye blind 3; or the side of the missing eye, the other side of the eye abnormalities with severe low vision 2 ;
b) of severe bilateral eyelid ptosis (or severe deformity) with three or more blind eyes; or heavy eyelids drooping on one side (or major malformations), the eyes of the blind more than grade 3, 4 blind eye over the other; < br> c) level 4 or above blind eyes;
d) binocular vision nearly complete defect (diameter of less than 5.);
e) maxilla, mandible, tooth loss, and 24 above;
f ) binaural hearing disorder with extreme ear missing on one side (or severe deformity);
g) a great ear, hearing impairment, the other ear and severe hearing impairment, with the side of the ear missing (or major malformations), the other side of the ear Missing (or deformed) 50% or more;
h) ears with severe hearing impairment with bilateral ear loss (or severe deformity); or severely hearing-impaired ears with ear missing on one side, the other side of the ear severe deformities; < br> i) 80% of facial scarring.
4.3.3 thoracic spinal deformity due to serious injury, serious impact on respiratory function.
4.3.4 Neck Injury:
a) scar formation, neck Ministry of complete loss of activity;

4.3.5 Chest Injury:
a) lobectomy or extensive pleural adhesions or thoracic deformity, seriously affecting the respiratory function;
b) heart failure, cardiac arrhythmia grade Ⅱ with organic; or cardiac arrhythmia class Ⅰ with significant organic.
4.3.6 Abdominal Injury:
a) the stomach, intestines, digestive gland and some other excision, digestion and absorption dysfunction;
b) the side of nephrectomy or total loss of the other side of moderate renal impairment; or bilateral severe disorders of renal function.
4.3.7 Pelvis Injury:
a) or complete absence of female ovaries atrophy;
b) fecal and urine incontinence, and difficult to restore.
4.3.8 perineal damage or total loss caused by bilateral testicular atrophy.
4.3.9 Physical Injury:
a) missing the second limb (upper limb above the wrist, leg above the ankle);
b) lack of a limb (arm above the elbow, legs above the knee ), another 50% loss of function of limbs;
c) absence of a limb (upper limb above the wrist, leg above the ankle joint), complete loss of function of the other limb;
d) total loss of a limb The other loss of function of more than 50%.
4.3.10 scarring of the skin damage caused by more than 60% body surface area.
4.4 Ⅳ-level disability
4.4.1 brain, spinal cord and peripheral nerve injury induced :
a) moderate intellectual impairment (IQ of 49 or less) or mental disorders, severely limited activities of daily living, sometimes need help;
b) severe motor aphasia or severe sensory aphasia;
c) quadriplegia (two limbs less than 4 strength);
d) hemiplegia or paraplegia (muscle strength 4 the following);
e) complete loss of erectile function.
4.4.2 Craniofacial Injury :
a) side of the missing eye, low vision other eye level 2; or the side of the missing eye, the other side of the eye abnormalities with severe low vision 1;
b) of severe bilateral eyelid ptosis (or severe deformity ) with low vision, two eyes above; or heavy eyelids drooping on one side (or major malformations), the eyes of two or more low vision, and the other eye blind three or more low;
c) 3 or more blind eyes;
d) binocular vision extreme defect (diameter less than 10.);
e) extremely deaf ears;
f) extreme in one ear, hearing impairment, the other ear disorder with severe hearing loss side of the ear (or deformity) 50%;
g) hearing-impaired ears with severe side ear missing (or severe deformity);
h) middle ears with severe hearing impairment with bilateral ear loss (or severe deformity); or binaural hearing impairment associated with moderate and severe ear missing on one side, the other side of the ear severely deformed;
i) more than 60% of facial scarring.
4.4.3 thoracic spinal deformity due to serious injury healing, affecting respiratory function.
4.4.4 Neck Injury:
a) scar formation, the neck more than 75% loss of activity;
b) affect the respiratory and swallowing function.
4.4.5 Chest Injury: < br> a) lobectomy or pleural adhesions or thoracic deformity, affecting respiratory function;
b) significant organic arrhythmia.
4.4.6 abdominal injuries caused by severe disorders of renal function on one side, the other side of the renal function moderate barriers.
4.4.7 perineal damage or complete absence of penis caused severe deformities.
4.4.8 vulva, vaginal injuries caused by vaginal atresia.
4.4.9 complete absence of physical damage or loss caused by hands function.
4.4.10 scarring of the skin damage caused by more than 52% body surface area.
4.5 Ⅴ level disability
4.5.1 brain, spinal cord and peripheral nerve damage caused by:
a) in the degree of mental defect or mental disorder, significantly limited activities of daily living, need guidance;
b) traumatic epilepsy, drugs can not completely control, large seizures more than once every March or limitations, or two or more seizures per month Little more than four times average weekly attacks or psychomotor seizures more than once per month;
c) severe apraxia or agnosia;
d) severe unilateral facial paralysis, difficult to restore;
e ) hemiplegia or paraplegia (more than one limb muscle strength of two less);
f) single-paralysis (muscle two less);
g) stool or urine incontinence, and difficult to restore.
4.5.2 head Facial Injury:
a. side of the eye with the other eye missing a low vision; side of the missing eye abnormalities associated with serious side of the eye and vision close to normal;
b. severe bilateral eyelid ptosis (or severe deformity ) with bilateral low vision 1; or heavy eyelids drooping on one side (or major malformations), which is more than one eye low vision, low vision other eye two or more;
c. low vision 2 eyes above;
d. binocular vision and severe defect (diameter less than 20.);
e. genioglossus complete paralysis or loss of tongue (or severe deformities) 50% or more;
f. maxilla, mandible, teeth off 20 or more;
g. extreme in one ear, hearing impairment, the other ear and severe hearing impairment;
h. ears with severe hearing impairment with the side of the ear missing (or misshapen) 50% or more;
i . binaural hearing impairment associated with moderate and severe ear missing on one side (or severe deformity);
j. bilateral ear loss (or major malformations);
k. completely outside the nasal defect (or severe deformity); < br> i. more than 40% of facial scarring.
4.5.3 Injury of thoracic spinal deformity union, affecting respiratory function.
4.5.4 Neck Injury:
a. scar formation, neck more than 50% loss of activity;
b. affect respiratory function.
4.5.5 Chest Injury:
a. lobectomy or pleural adhesions or thoracic deformity, a slight influence on respiratory function.

4.5.6 Abdominal Injury:

4.5.7 Pelvis Injury:
a. missing or bilateral ureteral atresia;
b. cystectomy;
c. urethral closure;
d. stool or urine incontinence, and difficult to restore.
4.5.8 perineal damage caused by missing most of the penis (or abnormal).
4.5.9 vulva, the vaginal stenosis vaginal damage, severe functional obstacles.
4.5.10 physical damage caused by:
a. hands missing (or loss of function) more than 90%;
b. Lack of a limb (arm above the elbow, legs above the knee);
c. Lack of a limb (upper limb above the wrist, leg above the ankles), another 50% loss of function of limbs;
d. a complete loss of limb function.
4.5.11 skin damage caused by scar formation of more than 44% body surface area.
4.6 Ⅵ level disability
4.6.1 brain, spinal cord and peripheral nerve damage caused by:
a. moderate mental defect or mental disorder, activities of daily living some limited, but could partly compensate, some daily needs help;
b. severe alexia with agraphia; or moderate or moderate motor aphasia sensory aphasia;
c. hemiplegia or paraplegia (one limb muscle strength grade 3 the following);
d. Single paralysis (muscle strength grade 3 the following);
e. erectile severe dysfunction.
4.6.2 Face and Head Injury:
a. The other side of the eye with visual acuity loss near normal; or absence of one side of the eye with the other side of the eye severely deformed;
b. severe bilateral eyelid ptosis (or severe deformity) with near normal binocular vision; or side of the eyelids with severe ptosis (or major malformations), which is near normal visual acuity, low vision one another above the eye;
c. a binocular low vision;
d. moderate binocular vision defects (diameter less than 60 .);
e. temporomandibular joint ankylosis, trismus;
f. extreme in one ear, hearing impairment, the other ear moderate and severe hearing impairment; or both ears with severe hearing impairment;
g. an ear Profile deletion (or major malformations), the other side of the ear missing (or misshapen) 50% or more;
h. facial scar area of more than 20%;
i. large number of small facial scar (or significant changes in pigment) 75 % or more.
4.6.3 cervical or lumbar spinal injuries caused by severe malunion, neck or waist, complete loss of activity.
4.6.4 neck injury caused by scar formation, the neck more than 25% loss of activity.
4.6.5 To the side of the abdominal injury and severe renal function disorder, the other side of mild renal disorders;
4.6.6 Pelvis Injury:
a. bilateral tubal absence or atresia;
b. hysterectomy.
4.6.7 perineal damage caused by bilateral vas deferens absence or atresia.
4.6.8 vulva, the vaginal stenosis vaginal damage and dysfunction.
4.6.9 Physical Injury :
a. hands missing (or loss of function) more than 70%;
b. lack of feet above the tarsometatarsal joint;
c. Lack of a limb (upper limb above the wrist, leg above the ankle joint ).
4.6.10 scarring of the skin damage caused by more than 36% body surface area.
4.7 Ⅶ level disability
4.7.1 brain, spinal cord and peripheral nerve damage caused by:
a. light degree of intellectual impairment (IQ below 70) or psychiatric disorder, daily life activities related to the ability to severely restricted;
b. traumatic epilepsy, drugs can not completely control, large attack in June more than once per episode, or the limitations of average February, more than twice per week, or two or more small seizures or psychomotor seizures more than once every February;
c. moderate or moderate apraxia agnosia;
d. severe dysarthria ;
e. hemiplegia or paraplegia (one limb muscle strength 4);
f. Single paralysis (muscle 4);
g. half-length or partial body type of complete sensory loss.
4.7.2 Head and Face Injury:
a. side of the eye missing;
b. severe bilateral eyelid ptosis (or severe deformity);
c. oral or temporomandibular joint injury, severe limitation of mouth opening ;
d. maxilla, mandible, tooth loss, and 16 above;
e. extreme in one ear, hearing impairment, the other ear moderate hearing impairment; or one ear and severe hearing impairment, the other ear moderately severe hearing impairment;
f. ear missing on one side (or major malformations), the other side of the ear missing (or misshapen) 10% or more;
g. Most of the nose outside the defect (or defects);
h. facial scarring, over an area of 24 square centimeters;
i. large number of small facial scar (or pigment change significantly) more than 50%;
j. over 75% of the scalp without hair.
4.7.3 cervical or lumbar spine injuries caused by malunion of the neck or waist more than 75% loss of activity.
4.7.4 anterior triangle neck injury caused by scar formation of more than 75%.
4.7.5 Chest Injury:
a. Lack of women with bilateral breast (or severe deformities);
b. heart failure, cardiac function grade Ⅱ.
4.7.6 Abdominal Injury moderate bilateral renal disorders.
4.7.7 pelvic Injury:
a. pelvic tilt, leg length difference of 8cm or more;
b. female pelvis major malformations, birth canal destruction;
c. absence or atresia the ureter on one side, and the other lateral ureteral stenosis.
4.7.8 perineum Injury:
a. missing part of penis (or deformity);
b. foreskin injury, scar formation and dysfunction.
4.7 .9 Limb Injury:
a. hands missing (or loss of function) more than 50%;
b. hand and feel completely missing;
c. feet arch structures are completely destroyed;
d. one foot above the tarsometatarsal joint loss;
e. leg length difference of 8cm or more;
f. loss of function of one limb more than 75%.
4.7.10 skin damage caused by scarring of body surface area of 28% or more.
4.8 Ⅷ-level disability
4.8.1 brain, spinal cord and peripheral nerve damage caused by:
a. mild mental impairment or mental disorder, daily life activities related to the ability of some restricted;
b. moderate alexia with agraphia;
c. half-length or partial body-type deep sense of loss;
d. erectile dysfunction.
4.8.2 Face and Head Injury:
a . a blind 4 or above;
b. a vision nearly complete defect (diameter of less than 5.);
c. maxilla, mandible, tooth loss, and 12 above;
d. one ear extreme hearing impairment; or an ear with severe hearing impairment, the other ear moderate hearing impairment; or moderately severe hearing-impaired ears;
e. side of the ear missing (or severe deformity);
f. tip of the nose and a side of the alar defect (or defects);
g. facial scar area of 18 square centimeters or more;
h. large number of small facial scar (or pigment change significantly) more than 25%;
i. 50 scalp without hair % or more;
j. maxillofacial bone or soft tissue defects of 32 cubic centimeters or more.
4.8.3 Spinal Injury:
a. malunion of cervical or lumbar spine, neck or waist more than 50% loss of activity ;
b. Second thoracic or lumbar vertebral compression fracture above.
4.8.4 neck injury caused by scar formation of the former Triangle more than 50%.
4.8.5 Chest Injury:
a . female side of the missing breast (or major malformations), the other side of the breast part of the missing (or misshapen);
b.12 than rib fractures.
4.8.6 Abdominal Injury:
a. stomach intestine, digestive gland and other parts removed, affecting digestion and absorption;
b. Splenectomy;
c. nephrectomy or renal side of the severe obstacles.
4.8.7 Pelvis Injury:
a. pelvic tilt, leg length difference of more than 6cm;
b. bilateral ureteral stenosis, or the side of the ureter missing (or closed), the other side of the ureteral stricture;
c. urethral stenosis.
4.8.8 Perineal Injury:
a. missing the glans penis (or deformity);
b. foreskin injury, scar formation, seriously affecting the function.
4.8.9 vulva, vagina Vaginal Injury narrow, severely affecting the function.
4.8.10 Physical Injury:
a. hands missing (or loss of function) more than 30%;
b. sensory loss over 75% hands;
c. a foot arch structures are completely destroyed and another damaged arch structure over 1 / 3;
d. toe complete absence of ten feet or loss of function;
e. leg length difference of more than 6cm;
f. loss of a limb features more than 50%;
4.8.11 scarring of the skin damage caused by more than 20% body surface area.
4.9 Ⅸ-level disability
4.9.1 brain, spinal cord and peripheral nerve damage caused by:
a. mild mental impairment or mental disorder, some limitation in daily activities;
b. traumatic epilepsy, drugs can not completely control grand mal seizures once a year or more, or the limitations of more than three times average or smaller attack in June more than four times per month, or psychomotor June more than twice per episode;
c. severe dyslexia or severe agraphia;
d. mild bilateral facial paralysis, difficult to restore;
e. half-length or light side of the body type of sensory loss;
f. serious impact on erectile function.
4.9.2 Face and Head Injury:
a. a blind three or more;
b. bilateral ptosis (or deformity); or heavy eyelids drooping on one side (or severe deformity);
c. a great vision defect (diameter less than 10.);
d. maxillary, mandibular defects , tooth loss, eight or more;
e. dental damage, tooth loss, and 16 above;
f. oral or temporomandibular joint injury, moderate, limited mouth opening;
g. tongue missing (or deformity) ;
h. one ear and severe hearing impairment; or a middle ear and severe hearing impairment, the other ear moderate hearing impairment;
i. a missing ear gallery (or deformity) 50% or more;
j. side of the alar defect (or defects);
k. facial scarring over an area of 12 square centimeters, or more than 20cm facial scar-like lines;
l. small facial scar (or pigment change significantly) area of 30 square centimeters above;
m. more than 25% of the scalp without hair;
n. maxillofacial bone and soft tissue defects of 16 cubic centimeters or more.
4.9.3 Spinal Injury:
a. malunion of cervical or lumbar , neck or waist more than 25% loss of activity;
b. a thoracic or lumbar vertebral fractures.
4.9.4 Neck Injury:
a. severe hoarseness;
b. anterior triangle more than 25% of scar formation.
4.9.5 Chest Injury:,
a. lack of female side of the breast (or severe deformities);
b.8 rib fractures or 4 or more Missing ribs above;
c. lobectomy;
d. heart failure, heart function class Ⅰ.
4.9.6 Abdominal Injury:
a. stomach, intestine, digestive gland and other partial ;
b. cholecystectomy;
c. Resection of the spleen;
d. side of the partial nephrectomy or renal moderate obstacles.
4.9.7 Pelvis Injury:
a. pelvic tilt, leg length difference of more than 4cm;
b. severe pelvic malunion;
c. urethral stricture;
d. partial cystectomy;
e. side of the deletion or blocking the ureter ;
